Fixing booking form

Hi there,

Where can I fix (or add & remove) the sections in the form on the right side of single tour page.

I would like to merge first name and last name in one column and would like to add dropdown menu in order to show the available pick up places.

I need to set separate option for the each tour. (eg. some tours need to have pick up dropdown box, but some tours are not.) Is there an any way to do this? (just like contact form 7, we can easily add dropdown box but still need to connect woocommerce system)
