Modify Contact Us Form

Hi,
I need your help to add two new field to the Contact Us Form,

1) Number Of Days (Indicating Number Of Days in Srilanka) must be a text Field
2) Budget (It Should be Dropdown) and the values should be
a) 15k – 20k
b) 20k – 30k
c) 30k – 40k
d) 40k & above
