Add date selection to booking form

How do I remove or add the fields in the form of the booking now?
I have entered in several dates for
choice of date – I would not like the calendar but groups of dates that i insert
is it possible to associate an id with the pulsane booking now? so i create a booking now button (in mobile version) that scrolls to the booking form.
