Form enquiry

Hi, when I enable Show Only Form Send Enquiry, the form does not show up on my product page. Why? Should not the form be visible?

I want the form to be there, but instead people buying the product, I want the form to send the enquiry to my email.

Also where can I set the email where the form should send the enquiry?
