Page title and description problem

Hi,
I am having an issue with the page title and description of one of my pages being copied from another page. The page title on the actual page does not display at all.
The page in question /tours gets its title from /jordan-blog
Also if I click on a blog article, I sometimes I get linked to /category/private-tours/, which does not make sense
Can you please advise how this can be fixed?
