Tour contact form subject code

Hi, On each Tour contact form we would like 4 fields:
Name

Email

No. of adults & children

Intended Departure

Also, we’d like each tour submission to have its’ own individual subject (the name of the tour) so the tour company know which tour the visitor was inquiring about.
What is the code for that and where to enter?
