Contact Form 7 – Error files

Hi, I install the template and activate the necessary plugins but i have a problem to activate contact form 7 so the solution that i find was delete conctact form and reinstall but the form [contact-form-7 id=”455″ title=”Send enquiry”] is not in my forms. Can you help me? Thank you!
